Registering as a New Adult Patient from Overseas
if you have moved to the UK and this is the first time you are registering with a GP please also send a copy of your photo ID (including resident permit or visa) and proof address via the document upload section
Registering a Child Patient: Immunisation History Required
If you are registering a child, please provide a copy of their immunisation history via the document upload section. In order to fully register your child at the surgery we will be contacting you to book a new patient health check
